What is Clinical Research?
Research in which people, or data or samples of tissue from people, are studied to understand health and disease. Clinical research helps find new and better ways to detect, diagnose, treat, and prevent disease.

Clinical research refers to all research carried out on humans (healthy
or sick people). It focuses on improving knowledge of diseases, developing
diagnostic methods and new treatments or medical devices to ensure better
patient care.
The
evolution of clinical research traverses a long and fascinating journey. From
the first recorded trial of legumes in biblical times to the first randomized
controlled of trial of streptomycin in 1946, the history of clinical trial
covers a wide variety of challenges - scientific, ethical and regulatory.
Clinical Trial
A clinical study in which participants are assigned to receive one or more interventions (or no intervention) so that researchers can evaluate the effects of the interventions on biomedical or health-related outcomes. The assignments are determined by the study protocol. Participants may receive diagnostic, therapeutic, or other types of interventions.
Different types of clinical research
are used depending on what the researchers are studying. Below are descriptions
of some different kinds of clinical research.
Treatment Research generally involves an intervention such as medication, psychotherapy, new devices, or new approaches to surgery or radiation therapy.
Prevention Research looks for better ways to prevent disorders from developing or returning. Different kinds of prevention research may study medicines, vitamins, vaccines, minerals, or lifestyle changes.
Diagnostic Research refers to the practice of looking for better ways to identify a particular disorder or condition.
Screening Research aims to find the best ways to detect certain disorders or health conditions.
Quality of Life Research explores ways to improve comfort and the quality of life for individuals with a chronic illness.
Genetic studies aim to improve the prediction of disorders by identifying and understanding how genes and illnesses may be related. Research in this area may explore ways in which a person’s genes make him or her more or less likely to develop a disorder. This may lead to development of tailor-made treatments based on a patient’s genetic make-up.
Epidemiological
studies seek to identify the patterns,
causes, and control of disorders in groups of people.
Phases of clinical trials when
clinical research is used to evaluate medications and devices
Clinical trials are a kind of clinical research designed to evaluate and test new interventions such as psychotherapy or medications. Clinical trials are often conducted in four phases. The trials at each phase have a different purpose and help scientists answer different questions.
Phase I trials
Researchers test an experimental drug or treatment in a small group of people for the first time. The researchers evaluate the treatment’s safety, determine a safe dosage range, and identify side effects.
Phase II trials
The experimental drug or treatment is given to a larger group of people to see if it is effective and to further evaluate its safety.
Phase III trials
The experimental study drug or treatment is given to large groups of people. Researchers confirm its effectiveness, monitor side effects, compare it to commonly used treatments, and collect information that will allow the experimental drug or treatment to be used safely.
Phase IV trials
Post-marketing studies, which are conducted after a treatment is approved for use by the FDA, provide additional information including the treatment or drug’s risks, benefits, and best use.
Big data and the digital revolution

As
the world changes and technologies develop and improve, new kinds of research
become possible. We live in a modern digital age where we have the possibility
of collecting and studying huge amounts of data. The era of big data is here
and it offers researchers in health care previously undreamt of opportunities.
But new possibilities also bring new challenges. What about our privacy? What
about choice? We must remember that research is about people and involves
people. Ethical studies and the rights of people involved in a study are
important areas we are going to pick up in the next section of the course.
This emerging technology and the
opportunities it affords us in clinical research. The world is changing rapidly
and with that change come new opportunities and new challenges. As more and
more personal medical information becomes readily accessible in digital
formats, new opportunities present themselves for research. In our new digital
age, we have an alternative approach, the so-called virtual trial.
Large
scale DNA bio banking studies are currently being conducted throughout the UK.
These will make available to clinical researchers massive amounts of genetic
data, supported and enriched by personal medical information. This availability
means that more than ever before, new clinically important discoveries will be
in the hands of desktop researchers rather than those working in the clinic. Modern
technology gives us all kinds of new ways that we could think about using
health data and health research data.
